What is it all about?
The New York Chiropractic College (NYCC) and Seneca County Public
Health Department (SCPHD) are partnering to improve the health and
level of
physical activity among Seneca County Residents.
Complimentary 6-month memberships to the NYCC will be offered to individuals and/or families who meet specific criteria. In addition to full use of all the facilities at the NYCC, nutritional counseling services will also be available.
What is involved?
Anyone can refer themselves and/or family. Once the referral is received
at the Seneca County Health Department you will receive a phone
call to discuss what is available in more detail including the
expectations to participate. A brief intake assessment for readiness
to change
and increasing physical activity, as well as, health questions
will
be asked during the phone interview. Referral forms are available
on the Seneca County Web site www.co.seneca.ny.us once
on the web site click on the “quick link” at the bottom
of the page or the Health Department’s ( box in red ) will take
you directly to the site of the referral forms. Referral forms
are also available at the Seneca
County Health Department, 31 Thurber Drive Waterloo or by calling
(315) 539-1920 or by printing and sending the form located at the
link below.
Commitment involves active participation in the program which includes use of the athletic facility at NYCC at least 10 times a month with an initial nutritional consultation and three nutritional follow-up progress visits. Free nutritional certificates are also available separate from the Athletic Center. If these criteria are not adhered to membership may be subject to discontinuation.
Participants chosen who meet eligibility requirements will also participate in a bio-metric screening before starting at the NYCC and upon completion of the program. This includes blood pressure, height and weight, BMI, fingerstick for glucose and cholesterol checks. Referrals will be kept at the health department for up to six months.
Confidentiality?
All information will remain confidential and if chosen to participate
HIPAA privacy consents will be obtained for disclosure of information
to NYCC and your primary care physician for evaluation of the progress
of the program. Referrals will be kept with the health department
for up to 6 months.
How long will the program be available?
A limited supply of free vouchers for the program is being offered,
however, the expectation is to have the program be ongoing depending
upon the joint agreement between Seneca County and NYCC. The vouchers
are for a six month commitment and if progressing well membership
can be extended for a full year.
